Sam actually digs for it, really wanting to know if he was the main theme of Dean's soul distress.
Sam: Hey, you said the Soul Eater made you see things. Plural. So⦠what else did you see? Dean: I saw you. Dead on the floor. Sam: Hmm. Dean: What? Sam: How messed up are our lives that you seeing a vision of dead me is actually kind of comforting? Dean: Yeah.
Because Sam is comforted by the reassurance that he is part of Dean's soul. That what the Soul Eater showed Dean to put him in a state of distress was Sam dead on the floor, not anyone or anything else, Sam.
This calls back to what I said about Love Hurts (11x13) and how weird that last scene of that episode hits. Because it really felt like Sam was bothered that the Qareen appeared to Dean as Amara and not... I don't know, him? And now he is relieved that the Soul Eater at least verified his place as Most Important to Dean.
